The horizontal roll test (also known as the supine roll test) is a diagnostic test designed to reveal the presence of Horizontal Canal Benign Paroxysmal Positional Vertigo or HC-BPPV. HC-BPPV is one type of Benign Paroxysmal Positional Vertigo and typically manifests as intermittent episodes of dizziness or imbalance resulting from changing the position of the head. You can expect your horizontal roll test at the Harley St AVM clinic to proceed in the following manner:

-You will be given a chance to ask any questions before the test begins.

-Following history and other assessment, you will then be seated on the examination couch.

-You will be positioned lying down, face up on the examination couch.

-The doctor will then turn your head approximately 45° to the right and hold it there until nystagmus or other symptoms have settled down.

-Your head will then be returned to a normal position.

-After a moment of stillness, the doctor will then turn your head approximately 45° to the left, and again hold it there until any disconcerting symptoms subside before returning it to a centerline position.

-While your head is turned the doctor will check for signs of nystagmus.

-Your doctor may choose to do this manoeuvre with the help of specialist equipment such as video Frenzel goggles to take a detailed look at any nystagmus.

If nystagmus is detected, a diagnosis of HC-BPPV will likely be made. If you are positive for nystagmus when your head is turned, the pattern of nystagmus has to be assessed by the specialist doctor who will then determine the side and type of BPPV. If nystagmus is not detected in either direction, your vertigo may be the result of something other than HC-BPPV.
