What is it?

Stapedial myoclonus is spasming of the stapedius muscle of the middle ear. The spasms cause rhythmic clicking, tapping, or fluttering sounds, which may be spontaneous or triggered by external or internal stimuli.

Typical Triggers include:

  • Loud noises or specific sound frequencies
  • Blinking, smiling, or other facial expressions
  • Psychological stress, fatigue, or sensory overload
  • Migraine-related sensitivity

Diagnosis

Requires detailed patient history and exclusion of palatal myoclonus or brainstem disorders. Audiological, imaging and neurologic assessments are key.

Long-term tympanometry or acoustic reflex decay can capture episodes objectively to confirm diagnosis

Treatment Options

Migraine prophylaxis medications or muscle relaxants

Psychological intervention such as CBT

Rarely, botulinum injections or surgery
