Overview

Tensor Tympani Syndrome presents with overactivation of the tensor tympani muscle in the middle ear.

Symptoms

  • Intermittent fluttering, clicking, or thumping in the ear
  • Pressure, fullness, or mild ear pain without visible inflammation
  • Hypersensitivity to sound

Mechanism
Tensor Tympani Syndrome is believed to result from a dysfunctional brainstem reflex pathway, where emotionally triggered exaggerated reflex responses occur via the limbic system.

Diagnosis

Requires detailed patient history and exclusion of other conditions including brainstem disorders. Audiological, imaging and neurologic assessments are key.

Management

  • Desensitization techniques
  • Cognitive behavioural therapy (CBT)
  • Sound therapy
  • In severe cases, medications to reduce symptoms.
