Horizontal Roll Tests in London

The horizontal roll test (also known as the supine roll test) is a diagnostic test designed to reveal the presence of Horizontal Canal Benign Paroxysmal Positional Vertigo or HC-BPPV. HC-BPPV is one type of Benign Paroxysmal Positional Vertigo and typically manifests as intermittent episodes of dizziness or imbalance resulting from changing the position of the head. Harley St AVM provide reliable and affordable horizontal roll tests in London at our state-of-the-art clinic.

Horizontal Canal Clarity Check

Roll, Reveal, Diagnose

You can expect your horizontal roll test at the Harley St AVM clinic to proceed in the following manner:

-You will be given a chance to ask any questions before the test begins.

-Following history and other assessment, you will then be seated on the examination couch.

-You will be positioned lying down, face up on the examination couch.

-The doctor will then turn your head approximately 45° to the right and hold it there until nystagmus or other symptoms have settled down.

-Your head will then be returned to a normal position.

-After a moment of stillness, the doctor will then turn your head approximately 45° to the left, and again hold it there until any disconcerting symptoms subside before returning it to a centerline position.

-While your head is turned the doctor will check for signs of nystagmus.

-Your doctor may choose to do this manoeuvre with the help of specialist equipment such as video Frenzel goggles to take a detailed look at any nystagmus.

If nystagmus is detected, a diagnosis of HC-BPPV will likely be made. If you are positive for nystagmus when your head is turned, the pattern of nystagmus has to be assessed by the specialist doctor who will then determine the side and type of BPPV. If nystagmus is not detected in either direction, your vertigo may be the result of something other than HC-BPPV.

I was seen by audiologist and Dr Arun, they were attentive, caring and took time to listen and adaptations were made throughout the appointment to support my anxiety and claustrophobia. This made a significant difference and genuinely helped me through the audiology testing. I gave an explanation of my current symptoms to Dr Arun he displayed great listening, compassion and understanding. The treatment I received on the day has greatly improved my symptoms, the one reassuring thing is any follow up is required is knowing that I will be in safe hands. I received my clinic letter promptly. After care queries handled by the admin team was exceptional, prompt and helpful. Overall the service was impeccable.

I had an outstanding experience with Harley Street Vestibular Clinic and cannot recommend them highly enough. I developed severe vertigo while visiting London, and it was an incredibly frightening experience as it was my first time ever experiencing dizziness like that. I called the clinic on a Saturday, fully expecting to leave a voicemail since they were closed. To my surprise, the office manager, Preeti, returned my call that same day. I was so grateful. She was kind, reassuring, and went above and beyond to arrange an appointment for me first thing on Monday. From the moment I arrived, the entire team was professional, efficient, and compassionate. They understood how urgent and distressing my symptoms were and made every effort to see me as quickly as possible. The vestibular audiologist was exceptional—she took the time to thoroughly evaluate me, clearly explain everything she was doing, answer every one of my questions, and perform the appropriate treatment. I never felt rushed, and her expertise immediately gave me confidence that I was in the right hands. The clinic was incredibly responsive, organized, and genuinely caring throughout the entire process. They turned what was a very scary situation into one where I felt supported and well cared for. I am truly thankful to Preeti and the entire team for their kindness, professionalism, and outstanding care. If you are experiencing vertigo, dizziness, or balance problems, I would not hesitate to recommend Harley Street Vestibular Clinic.

Dr Benjamin Shaw was extremely thorough in his assessment of my recent attacks of vertigo. I have total confidence in his ability, once I have completed the further tests he has arranged at the Wimbledon clinic, to diagnose what is causing this issue with my health.

Pinpointing the Cause

Head-Turn Balance Decoder

Precision Rolls for Vertigo Insight

Knowing when to seek help matters. While sensorineural hearing loss is often gradual and manageable, certain patterns signal the need for prompt specialist review to rule out treatable or serious causes.

You should get assessed if hearing changes occur suddenly, affect one ear, are accompanied by pulsating sounds, severe dizziness, facial weakness, visual disturbance, new neurological symptoms, or follow infection, head injury, or new medication use. Prompt review ensures timely diagnosis and appropriate management of early care.

Horizontal supine roll test to assess positional vertigo at Harley HVM

Gold-Standard HC-BPPV Detection

At Harley AVM, assessment starts with a detailed clinical history to understand how hearing has changed, whether symptoms are gradual or sudden, and how they affect communication and daily life. We explore associated features such as tinnitus, imbalance, noise exposure, medical history, and medication use. This step helps us decide which investigations are appropriate and avoids unnecessary testing.

Depending on symptoms, assessment may include pure-tone audiometry, speech audiometry, speech-in-noise testing, tympanometry, acoustic reflex threshold measurement, eustachian tube function testing, and otoacoustic emissions. All testing is reviewed with our senior audiovestibular clinician, ensuring results are interpreted within the full clinical picture.
